are there any guidelines for the thermal shrinkage on knees, hips, shoulders? my physician has a problem when i dont bill for every procedure with 29999 based on the shrinkage. I remember reading something about guidelines for documentation but i can't find it. I will meet with him tomorrow so any help? advice?
