Wiki Third Party Billing Agency State Certification/Registration

I currently do business in New Jersey, and as of 2005 all third party billing services are required to be certified by the NJ State Department of Banking and Insurance to be able to conduct business in this state. I'm doing some consultant work with a law firm, and they wanted to know if other states, specifically the ones listed below, have to certify with their state to conduct business as a third party billing service/agency.

Connecticut
Florida
Georgia
Illinois
Kansas
Maryland
Massachusetts
Michigan
Minnesota
Missouri
New Hampshire
New York
Pennsylvania
Rhode Island

Any information that anyone has regarding any of these states would be greatly appreciated. Thank you.
