Since this is a time-based code (first 30 minutes, face to face..), is it appropriate to document "greater than 15 minutes spent with patient", or must the record reflect the specific amount of time provided for the service? I'm trying to find a specific reference to this.
 
I believe in the Medicare regulations, and the federal register it is stated that for timed based services the total face to face time spent with the patient must be documented by the provider. I have come across a few commercial payers that state it is their policy that this be documented in terms of time in and time out clock time.
