From the time service is rendered, what is the required time frame the provider must sign off on the medical record?

This varies from practice to practice. Our policy is that all charts should be signed the same day or within 24 hours, but I have worked with clinics that allowed the providers up to 3 days to sign their charts. Check your clinic's policy, it should be listed somewhere. If not, talk to the clinic manager to have it added to the policy. It is vital to have a clear policy that you can fall back on when trying to get the providers to sign their charts. It is very difficult for the billing office to get the providers to sign in a timely manner if there is no set policy to reference.
