Hello, I may be thinking this through too much but wanted to ask if you know if when coding based on time does the note have to be signed off by provider on same day?
 
In general no. May be service dependent if a particular code has a specific requirement. However, for E/M not really. Signing ASAP/same day/at the time of completion is always best.
