Our ENT MD sees patient in hospital for first time.
She spends an hour at bedside.
Her History is not Comprehensive, so she only scores a 99221 for MDM, 1995 guidelines..
But CPT says 50 minutes is usual amount of time spent at bedside for a 99222.
What should documentation look like for a hospital E/M to be 99222 for TIME FACTOR?

Inpatient E/M codes still follow the old guidelines: in order to use time to determine the level, the documentation must reflect that more than 50% of the total time was spent in counseling and/or coordination of care.
