Wiki Timeliness of documentation

Does anyone have any links to regulations regarding the timeliness of medical record documentation? Having trouble with some staff, at all levels, not completing records for weeks, or in some cases, even longer. Examples - medical assistant draws blood, and documents it three weeks later. Nurse gives penicillin injection and documents it a week later. Physician does a new patient exam and documents it a month later.

Timeliness of documentation - Medicare intermediary guidance

I found two documents from Medicare intermediaries on this subject and both state that documentation should be completed within 2-3 days at most:
