I have been asked to assist in creating a template for these procedures. does anyone have examples they would be willing to share?

Hello. What sort of Template are you creating. Dont know that I can help, but I also have a question. I bill professional services for hospitalists @ a Behavioral Health Hospital and we are going to start offering this treatment. Psychiatrist will bill 90867 for initial. However, subsequent visits will be preformed by a technician under the supervision of the psychiatrist. Do we need to submit the 90868 with a modifier? If so, which would you recommend
 
I have a board certified Emergency Physician that's retired and want to do Transcranial Mag Stimulation in his private office. Do these codes require a psychiatrist or can an ED doc perform these procedures for Medicare and the Commercial carriers?
