Wiki Toe Grafts with Partial Amputation

My doc did partial amputations (28820 or 28288) of the 1st and 2nd toes and a FTSG on each of the 1st and 2nd digits. He said he had to create two separate grafts. Do you bill for 2 separate grafts as 15240 and 15240-59 or do you add the sum of the areas. He insists that he should be paid separately. Can you also bill the 15004 for preparation of the site.

The skin grafts are coded by the size and location of the graft, not by the number of grafts used. So you will need to know the total square centimeters of the graft/ grafts and where they are applied to determine which code to use.
