In desperate need of help!!! I have never done podiatry before so I am needing help with some codes. This was done on a medicare patient who has DM with neuropathy and onychomycosis. The note reads:

"The patient's nails 1 through 5 bilateral were debrided with a nail nipper and nail grinder in length/width/thickness of both feet. The patient was educated on foot care regarding proper cleansing medication and appropriate shoe wear."

What CPT/HCPCS and modifiers would be used? Also, would you list the primary diagnosis as DM with neuropathy? Thanks in advance for any help.
 
I would recommend going to your local MAC website and review the local coverage determination. You can find it by searching for foot care.
For Oregon it is LCD L36404.
It will give you the billing codes, documentation requirements as well as Dx coverage.
You may want to also review coverage for G0127.
