My software rejected a claim stating there were too many diagnosis codes for the service rendered (elective abortion). I thought we could enter up to 12 codes. What is this about? None of my other claims had any problems. (this one was to UHC by the way.)
 
You can enter up to a total of 12 codes if field 21 however you can link only 4 of the 12 to an individual CPT code in field 24. For an elective abortion what's 12 codes did you list? Which ones did you link?
