doctor did a total hip replacement but also did prophylactic plating for her femur for a potential stress riser since she has had a revision in the past - how would you code this with the 27130?

Your query is a little confusing, particularly as to your reference to a "revision" in the past. What was "revised?" If it was a previous THR that was revised (before), and now revised again, then 27130 would not be correct as that applies to a primary THR, which does not appear to be the case here. If the current procedure is a "revision" of a THR, then you would have to look at 27134: Revision THR, both components; 27137: Revision THR, Acetabular Component only (unlikely); or 27138: Revision THR, Femoral Component only (more likely).
As for the prophylactic plating of the femur to prevent future stress fracture, the most likely location of such a "stress fracture" is at the distal end/tip of the Femoral Stem, which would be a femoral shaft fracture, and 27495: Prophylactic Treatment of Femur with plating, with or without cement. To this you would probably add Modifier 51 for Multiple Procedure.
Also send the Operative Report with your claim. Hopefully it has all the documentation content to justify his procedures.
