Did the physician state patient has CAD due to calcified coronary lesion? If not, I would instead look at 414.00/414.01. You can also look at 444.22/444.21, if your documentation meets m.n. for such dx.

Well 441.2 is for Thoracic aneurysm without rupture. Did the physician document where this total chronic occlusion is?

Are we talking coronaries or possibly peripheral arteries?
