If the provider injects 8 muscles in 1 arm, we should only bill 64644, correct? It would not be appropriate to bill 64644 and 64642 since all muscles are in the same extremity. But if the provider injects 5 muscles in the RIGHT arm and 3 muscles in the LEFT arm, would we bill 64644 with 64642 or 64643?
