TPA Installation

I have a patient that came into clinic and received a TPA installation, which is a substance that is infused into the patients port to break up any blockage that might be occuring. The patient is monitored until the nurse is able to retrieve blood flow out of port. My question is, how would you bill for this? Our thoughts are maybe billing 96365 for the initial hour, and 96366 for any additional hours. The description of these codes state specify substance or drug. Does anybody else bill for this service, and how do you bill?

We bill it as 36593 x 1 with a 59 modifier and use 996.74 as an additional dx code. Of course, we make sure there is sufficient documentation by the nurse to support it.
