Hello, I am looking for help with this one because I have not seen the tPA instillation done with a drainage before.

CT GUIDED DRAINAGE OF PELVIC FLUID COLLECTION

Initial CT images obtained with localizing grid over area of interest. Previously demonstrated collection was identified. Overlying skin was prepped, draped & anesthetized. Under CT guidance, an 18-gauge needle was advanced into the collection. Needle position was confirmed with aspiration of fluid. The needle was removed over an Amplatz wire and tract serially dilated. A 12 French all-purpose drainage catheter was advanced over the wire anpigtail formed. 15 cc of hemorrhagic fluid was aspirated and sample sent for microbilogy. Repeat CT images confirmed catheter position. The catheter was secured with a suture. A total of 6 mg of tPA was instilled via the catheter.

IMPRESSION: Successful CT-guided drainage of pelvic hematoma. tPA instilled via catheter with plan to connect suction drainage after 2 hours.
