Transfer of care during global period

A patient was seen initially in Florida for fracture care. She then returned to Michigan for a follow-up consultation. My doctor wants to bill an office visit, but which modifier do I use to show transfer of care from the original doctor in Florida?
 
You use the same fx care code the original provider billed with a 55 modifier. You need to put a transfer of care note in field 19 ( notes section) indicating that the care was transferred from that provider. Be sure you have a written transfer from the original physician in the chart. Also do not use a fracture dx code use only the V code for healing fracture.
