We are a nephrology practice. If one of our patients is in a hospital which we don't round, so we don't see patient while he/she is in hospital. After discharge we contact patient within 2 day requirement and see patient face-to-face within 7 to 14 days. Can we bill a transition of care service or should we bill a hospital follow-up visit since we didn't see the patient while in the hosptial?
