It has always been my understanding that unless a patient is actively having a CVA you always bill History of CVA. So if a patient comes in for a Transition of Care visit from being admitted for a CVA..do you use the diagnosis of CVA or History of?
 
Interesting question, and one that pertains to various other diagnoses, as well. For example, as dermatologists, we might continue to treat a wound caused by surgery to remove a skin cancer. So is the diagnosis (along with the open wound) still skin cancer, or is it history of skin cancer?

I have never seen a clear answer, and so would also be interested in any replies!
