Transition to Brain injury Rehab-Do I use an Initial or Subsequent hospital visit code?

If a patient is admitted for a traumatic brain injury for over a month, seen multiple times by Physical Medicine and Rehab specialty, then is discharged and readmitted the same day to an inpatient brain injury rehabilitation program, is it an initial or a subsequent visit for the admission day? The Phys Med and Rehab doctor is the admitting provider. Prior to being discharged the admitting dept was Hospital Medicine. Should the POS change from 21 to 61? It just seems strange that the same doctor saw the patient multiple times, and then is billing an initial code after they had billed a subsequent code in the days prior.
