I looking for any help with documentation guidelines for 99495/99496. Not sure if you have to includ all the non face-to face elements?? THANKS
 
CPT lists the documentation guidelines. Do a search on this board for Transitional Care Management. There are several huge threads on the topic which should address all of your questions. Pam
