I need some help understanding what date we are suppose to use for the date of contact on the transitional care worksheets? From the past webinars that I have attended they stated that the date of contact is the day the provider reconciled the patients meds, discussed treatment until they follow up with their primary care, ect. Our doctors in the office usually are the ones who discharge the patient and they tell them to follow up, so what is confusing is the fact that what if our doctor is not the one who discharges them and the patient's discharge summary is faxed over would that count if they go over it and use the date for that day? I know this sounds confusing so if someone can help me that would be wonderful!
