With the codes 99595 and 99596 we do not bill the TCM visit until the 30th day after patient's discharge, though we will actually have the visit long before the 30th day. Is the date submitted for the charge the date of the visit (but the charge was held until the 30th day), or do we use the 30th day as the visit date?
