Can anyone tell me which, if any, commerical payers are reimbursing for the Transitional Care Management Services?
 
There are some

Depending on your contracts/where you live so far I heard of:
UHC
Humana
WEA
WPS
Unity
Dean Health Plan
Network Health Plan
But it seems there is no guarantee they will pay, hope this helps Jessica.
