On Medicare CPT 99495 and 99496 when do I actually bill submit billing? Is it on the 30th day? Is the date of service the date the physician saw the patient in an office visit (within 7 or 14 days of discharge)? Not clear on these since we have just added several patients who qualify.

You would bill with the 30th day post discharge as the DOS

Remember that the Transitional Care or TCMS begins (includes) the day of discharge, so if patient was discharged on 10/1/13 then begin counting from 10/1/13 and you would bill for dos 10/30/13.
