I was wondering if you should bill 57288 with 57267 (for the mesh)? I just read were we can bill for both codes.
 
No, you should not. 57267 is an add on code and should only be used in conjunction with 45560, 57240-57265, 57285.
 
I see that note now under that code. Interesting the article that said 57267 could be billed is out of OBG Management October 2010 article.
