My doctor performed per his op note 2 bi-lateral TPI 20553-50 on L 4-5 & L5-S1 under fluoroscopic guidance with steroid injected. He injected these over hardware (screws) due to pain . Can we bill 2 trigger points ? Would we add a 59 or 51 to the 2nd code ?
 
Hi there, you can only report one trigger point. The codes in this group are based on the number of muscles injected. (One or two muscles OR three or more muscles):
  • 20552 Injection(s); single or multiple trigger point(s), 1 or 2 muscle(s)
  • 20553 Injection(s); single or multiple trigger point(s), 3 or more muscles
To take an extreme hypothetical, if the doctor treated trigger points in every muscle group in the patient's body, you would report 20553, once.
