I work for a large otho group who all bill under the same tax ID #. They frequently decide to do surgery and need a pre-op eval done by another provider in the same group. We keep getting denials saying we cannot bill for two visits the same day. We have tried different modifiers to no avail. Any Ideas anyone. :(
 
The initial E&M can be billed with a 57 modifier, decision for surgery, but the H&P is part of the surgery package and cannot be billed separately.
