Good afternoon,

I have a quick question.
Scenario:
A. Right breast, 2 o'clock, needle biopsy: Dx: Invasive Ductal Carcinoma
B. Right breast, 4 o'clock, needle biopsy: Dx: Invasive Ductal Carcinoma

Would you code each quadrant separately, code the overlapping or code the unspecified primary site?

I happened to fall across this question today and before I go training something I am not even sure of, I thought I'd ask for your guidance. I read today in the "seercancer.gov" the following which threw me off completely. Your thoughts are appreciated. Thank you.

SEER guides us to use C50.9xx with multiple sites but our payers our getting angry dealing with the dreaded unspecified with denials. We do not crystal-clear guidance on overlapping sites. Maybe they have updated their information? I would have billed both locations not counting on the unspecified code here in my coding opinion here.
