Post auricular incision is done and aerolar tissue is harvested for graft. Tissue is set aside and incision closed. Perforation is serially perforated and its rim of epithelium is removed and undersurface of TM is abraded with angle hook. Meatal incision is done from 12 to 6 o'clock. Tympanomeatal flap is elevated and middle ear is entered. Middle ear packed and graft is placed into position. Flap returned to anatomic position. Gelfoam applied over flap and canal filled with Bacitracin.

The provider performs a myringoplasty, or surgical repair of the eardrum, using graft material such as a piece of fascia or a plug of fat. The surgery involves only the drumhead of the tympanic membrane and the graft donor area. The surgery is performed to close a perforation of the eardrum and prevent infection.

Clinical Responsibility
When the patient is appropriately prepped and anesthetized, the provider uses a microscope or loupes, i.e., magnifying lenses, attached to glasses or a headband to locate the tympanic membrane defect. He roughens up the tympanic membrane perforation to prepare it for the graft, a technique also referred to as rimming the perforation. If needed, he places dissolvable packing in the middle ear. The provider uses either a fat graft plug or fascia to repair the hole. He sutures the incisions closed, instills antibiotic drops in the external auditory canal, places gauze soaked in antibiotic solution in the ear, and applies a dressing.
