We are an outpatient hospital facility. Many of our procedures are injections which require two codes such as 64483 and 64484. We have been entering code 64483 in the body of the claim, field 44. We then enter 64484 in box 74a. We're now being told this is incorrect. If so, where does the 64484 code get entered?
