Wiki UHC bundling issuse

I am having an issues with UHC claims bundling cpt 17004 and other destruction codes cpt 17280. I have been told that it is an inappropriate coding combo and have tried adding modifier 59 to cpt 17004 but they are still denying. I was thinking of adding modifier XS but am not sure. Any help would be appreciated. Thanks

UHC Bundling Issue

Does the documentation support use of the XS modifier? You can't just add a modifier without documentation that justifies its use as a distinct procedure.

my thought is that if it is in the same area via the same removal process as the malignant lesion then the removal of the non malignant lesions would be bundled. Only use the modifier if the removal was done in another area.
