Ok all my Arizona Peps, UHC Community plan is denying all my claims for HCPCs code S5125-U5 stating the modifier is missing, invalid or incorrect for the procedure code billed. This just start within the last month. I've been combing the internet on the AHCCCS & UHC CP websites and can only find literature that disproves this denial. These are LTC members. I'm appealing and so far one has reversed and paid, the others are pending. I don't want to appeal all of these so does anyone having this same issue as well? How are you handling it?
