I have several Physical Therapy claim that have been denied for "Processed According to Allowable" but I think this is an error.

We billed 1 unit of 97035 (GP) and three units of 97110 (GP). UHC paid the 1 unit of 97035 but denied the other three units. The diagnosis on these claims are 719.45, 729.5, 728.87 and 821.01. According to NCCI edits neither of these codes require a modifier and I'm stumped as to why they will not pay. Can anyone offer any suggestions on how I should proceed?
