Can anyone suggest a code for this ulcer on trunk? Exam as follows:

Skin: Right trunk with 2.5 mm to 3 mm superficial ulcer with granulation tissue. No surrounding erythema or purulence.

A/P: Improved appearance to wound. Continue to monitor.

Thanks for any help. I've checked the Index and not finding anything that seems to fit.
 
I would ask for clarification as to where on the trunk. Chest? Breast? Close to the axilla? Flanks? Back (posterior)? Abdominal area (anterior)? That should get you closer to the code you want.
