If you were coding a colonoscopy and got a path that read -
Moderate to severe chronic active colitis with focal surface erosion. No glandular dysplasia identified. See Comment.

*Diagnosis comment: The histologic changes throughout the colon are compatible with the patient's clinical history of ulcerative colitis. Clinical correlation and follow-up is suggested.*

What diagnosis would be appropriate? Differences of opinions in our office in the use of the diagnosis comment. Would chronic colitis, 558.9 be appropriate or would you just code the ulcerative colitis 556.9 from the diagnosis comment?
