Unbundling if a patient has no insurance

If a physician sees a patient in the office and also does an in-office trigger point injection and the injection is related to the E&M (bundled), can the physician bill the patient for both the E&M and injection if the patient does not have any insurance (no managed care contract, no insurance, a full self-pay patient) or does the physician’s billing office need to respect the bundling edits and only bill the patient according to CCI bundling edits?
