Wiki Uncontrolled DMI - Hyperglycemic

My provider is dictating uncontrolled DMI. He does not say if it is Hyperglycemic DMI. My question is, is it always assumed that uncontrolled Diabetes is Hyperglycemic or does the provider have to dictated Hyperglycemic? I know that he should dictate if it is Hyper or Hypoglycemic but does he have to dictated the words.
