DX: Status post ORIF of left hip with loss of fixation.

What would I bill if my surgeon performed:
1: Revision of a failed ORIF to a hemiarthroplasty
2: ORIF with internal fixation greater trochanter using a Zimmer tranchanteric
150 system.

I was thinking 27236-22 and 20680 but that doesn't seem right. Does anyone have any advice for me on this one because it seems like I could bill the 27236 twice, once for the ORIF and one for the hemiarthroplsty but that doesn't seem right either.

Any advice would be greatly appreciated.

Same hip? I see the 27236-22 since it was a conversion to a hemi. But was ORIF done first - then when it didn't take doc did hemi? same day?
