Per the UHC guidelines they only cover 10 units per arm. We bill out 95165 x 10 units with modifier RT and then 95165 x 10 with modifier LT. But they still only pay for one of the 95165 codes. Does anyone have any suggestions or help with this situation?? I appreciate any in advance.
