Should we append modifiers to an unlisted procedure code or only to the comparable codes?
Should we list an unlisted procedure code more than once on a claim for the same comparable code?

This is how I report unlisted codes : I only list the unlisted code ex. 29999 without any modifier and write a short description of the procedure in box 19 or the notes sec. of the claim page. Then I write a letter mentioning the comparable code and the reason for using the unlisted code and send it along with the claim.
