Question.... Do all the dx codes on a claim have to be ALL specified 4th, 5th, 6th.
 
Your question is unclear. However all codes must have their required number of characters as represented in the tabular section of the code book. Most payers are now d nying claims for codes that state unspecified. However if you have a logical reason for using unspecified then you can try to appeal these. Such as pneumonia unspecified, this code may deny, but it is totally logical to use she, the provider may not know the specific type of pneumonia sinc infections are often diagnosed without definitive workup and diagnostic testing. On the other hand , osteoarthritis of the knee unspecified is not logical. The provider is expected to document the type and which joint and laterally is involved.
