Wiki urgent......Billing e&m for both physician and nurse for psych services

I have a doubt in billing office visit. here in our clinic the nurse has documented every thing before the physician has seen the patient. the physician only performing assessment plan. here in this case can we bill any services for nurse, since all the service has furnished by nurse only except plan.
please anyone make me clear on this.
