Has anyone else been getting denials on Urinalysis when the pt has an office visit same day? We started appending a 25 modifier to the e/m or level of service but still getting denials?? I did not know is that is a service that insurance is considering included with the ov. Any suggestions or opinions would be help to me! thanks
 
Is this denial with a particular carrier? It could be that it's considered included in the e/m as you stated earlier. I know we have this issue with a particular carrier only.
 
thank you for you response. I beleive it is Medicare and BlueCare.. I am from TN and Bluecare is a government funded insurance.. I am not sure of any others at this time
