Medicare GA is rejecting an 81002 claim. I billed 99213-25, 81002-QW with diagnoses 788.1. Can someone help me!!!
 
According to another thread on this subject (I have been getting denials as well), Medicare no longer covers the UA separately. It is considered part of the E/M (99213 in your case). Bummer!
