Wiki urinary retention/ catheter reinsertion

A patient came to the office at 9:00 am to have her catheter removed as a follow up for urinary retention. At 3:00 pm the same day, she returned to the office unable to void and had the catheter reinserted by another physician. This is a new scenario to me, could someone please help with coding it?
