I am hoping that someone will be able to answer a coding question that we have for laboratory services specifically pertaining to the appropriate use of CPT code 80307.

We currently obtain a urine sample and a blood sample from our patients when indicated to perform various laboratory tests. Included in the urine testing is a drug screen for opiates, cocaine, barbiturates, and oxycodone. We will be billing the new code 80307 for this testing. Here is the actual language: “80307 - Drug test, presumptive, any number of drug classes, any number of devices or procedures, by instrument chemistry analyzers [e.g., utilizing immunoassay (e.g., EIA, ELISA, EMIT, FPIA, IA, KIMS, RIA)], chromatography [e.g., GC, HPLC], and mass spectrometry either with or without chromatography, [e.g., DART, DESI, GC-MS, GC-MS/MS, LC-MS, LC-MS/MS, LDTD, MALDI, TOF] includes sample validation when performed, per date of service”.

We also perform a blood (serum) level test for acetaminophen. We would like to utilize CPT code 80307 with modifier 59 for this test pursuant to separate instructions in the 2017 CPT manual which states “When the same procedure(s) is performed on more than one specimen type (e.g., blood and urine), the appropriate code is reported separately for each specimen type using modifier 59”.

Does anyone know if we really can bill 80307 for the urine drug screen and 80307 with modifier 59 for the serum acetaminophen test (same encounter/same date of service)?
