Urodynamic Documentation Requirements

I need help with find the documentation requirements for Urodynamic studies in the office. When the doctor just signs the computer print out/report is that considered their "interpretation" or does the provider need to document a separate written report with results? Any input or guidance on how it's done in your office would be great.
