Can someone please help me? I really need to understand this coding from my provider:

The patient was identified taken back to the procedural suite where prophylactic antibiotics were confirmed. The patient was then placed in the lithotomy position for cystourethroscopy. They were prepped and draped in the usual sterile fashion. A timeout was performed and the procedure is begun.
Procedure begins by introducing the cystoscope atraumatically into the urethra. Direct visualization is taken into the urinary bladder beyond the position of the posterior urethra. No abnormalities are noted.

Within the urinary bladder inflammatory lesions are noted to the bladder wall, dome, trigone inflammatory and treated with fulguration and controlled. Once this is done, the remainder of the circumferential examination reveals no other abnormalities such as stones or other malignant tumors. Bilateral ureters are seen effluxing clear urine.

Cystoscopy cystometrogram was then performed at 80 cm water pressure. Capacity is noted to be 420 cc.
There is not evidence of detrusor overactivity. Patient tolerated the procedure well. They are then taken to consultation room.
