Hello everyone, I have a new physician who is a urologist. He will be doing some tests in the office do I apply for Clia or a Clia waiver cert. Please help

Most likely you will only need a clia waiver certificate. Things to consider:

Only performing a dip urine test?-clia waiver
Performing a dip urine test and physician looking in a microscope?-need a ppmp clia certificate
